How the Family Practice Model Works

Register your interest in FPM by filling out the online form with your staff credentials
or create an account as a Staff Dependant. Provide basic details about yourself and your family to get started.
2. Doctor Assignment
Choose three (3) FPM Doctors who will be attending to your personalized care.

View your doctor’s availability and schedule appointments.
Manage your healthcare easily with a user-friendly scheduling system.
